Probationary or Conditional Reviews for Faculty and Academic Professionals
Material submitted to the dean shall include (see guidelines for Promotion and Tenure for Faculty or for Promotion and Continuing Status for Academic Professionals for more detail):
- The Request for Academic Personnel Action form (available online at http://provost.asu.edu/forms),
- Additional forms used by the academic unit.
- Table of Contents.
- A job description for the academic professional.
- A current curriculum vitae.
- Self-assessment - goal statement (4 page limit).
- Summary evidence of teaching and/or job effectiveness.
- Evaluation(s) by personnel committee(s).
- Transmittal letters of the chair/director.
- A current copy of the unit/department approved probationary review process criteria.
The dean will provide a cover list of all probationary reviews (or conditional reviews) of faculty and/or academic professionals in the college, listed by academic unit, who were approved for a regular contract or a conditional contract for the following year. Attach a copy of the dean’s notification letter to the candidate and the signed original Request for Academic Personnel Action form.
Note: distribution of notification letters to the successful candidates should be made at the same time as the notifications of decision from the provost on the recommended terminal contract reviews.
If the dean’s recommendation is for a terminal contract, the entire file should come forward to the Provost Office for review including the following:
- Candidate’s file.
- College committee’s recommendation letter.
- Dean’s recommendation letter.
- A copy of the college’s approved criteria for probationary (or conditional) reviews.
