Appendix 1: APR preliminary information form

Unit under review:

Units under review will be notified by the Office of the Provost in the spring semester ahead of their review cycle.  At this time, the unit director will identify the key participants for the review so that they can participate in an orientation session also held in the Spring semester.

Appendix 2: APR self-study report template

Self-study review committee:

Complete the APR Self-Study Report template. PLEASE NOTE: this template is currently under review and an updated version will be posted when complete. Units may use the currently published template to review for general content. More information will be discussed with 2026–27 cycle participants during the April 28, 2026, APR Orientation meeting.

Appendix 3: APR site visitor nomination form

Unit under review:

Complete one APR Site Visitor Nomination form per nominee.

NOTE:  Nominated site visitors must be approved by the dean.

Appendix 5: APR site visitor itinerary

Appendix 6: APR two-day sample site visit schedule

Unit under review:

Please download and use the APR Two-day Sample Site Visit Schedule as an example of a two-day visit.

Appendix 7A and 7B: APR site visit report template and APR signature page for site visit report submission

Site Visitor:

The UPRA Office provides an APR Site Visit Report template for your convenience. After completing your report, email it to the UPRA Office with all site visitors copied to indicate their agreement.
