Listed below are the appendices to the Arizona State University Academic Program Review Manual.

Appendix 1: APR preliminary information form

Unit under review:

Units under review will be notified by the Office of the Provost in the spring semester ahead of their review cycle.  At this time, the unit director will identify the key participants for the review so that they can participate in an orientation session also held in the Spring semester.

Appendix 2: APR self-study report template

Unit under review:

Complete the APR Self-Study Report template.

Appendix 3: APR site visitor nomination form

Unit under review:

Complete one APR Site Visitor Nomination form per nominee.

NOTE: Nominated site visitors must be approved by the dean.

Appendix 4A and 4B: Logistics letters

Unit under review:

After UPRA confirms the site visitors, the unit manages logistics for the site visit, including sharing the information provided in the following examples with the site visitors. Be sure to update all red text placeholders when customizing the template:

Appendix 5: APR two-day sample site visit schedule

Unit under review:

Example format for an APR Site Visit Schedule. Instructions for minimum meeting requirements are described in the "Site visit schedule" section of the ASU APR Manual.

Appendix 6: APR site visit report template

Site visitor:

The UPRA Office provides an APR Site Visit Report template for your convenience. After completing your report, email it to the UPRA Office with all site visitors copied to indicate their agreement.

PLEASE NOTE: this Site Visit Report template is currently being revised to align with the updated Self-Study Report template published in Appendix 2. This website will update with the new template as soon as it is confirmed.
